Presence homecare LLC is registered to provide 24 hours supervised services to the needy population. We provide comprehensive home care services to individuals with intellectual, physical or developmental disabilities in community-based home setting environments in most Twin Cities Metro areas of Minnesota. Presence Homecare is committed to enriching the community with compassionate and quality homecare service that people deserve.

Emergency Assistance
. 24 Hours emergency assistance
On-call counseling and problem solving and/or immediate response for assistance at a person’s or community home due to a health or personal emergency.

- Brain Injury (BI)
- Community Access for Disability Inclusion (CADI)
- Community Alternative Care (CAC)
- Developmental Disabilities (DD).
. Home Health Aid (HHA)
A qualified employee of Presence Homecare completes medically oriented task(s) that maintain a person’s health or facilitate treatment of an illness or injury. HHA services are provided in a person’s place of residence or in the community-based home.
Home Health Care
. Home Care Nursing (HCN)
Professional nursing services provided to a person who requires more continuous care than a skilled nursing visit (SNV) and beyond the scope of what personal care assistance (PCA) and home health aide (HHA) services can provide. HCN services are based on an assessment of the person’s medical/health care needs and are provided in the person’s place of residence or in the community. All HCN services must be ordered by a physician, physician assistant (PA) or advanced practice registered nurse (APRN).
. Skilled Nurse Visit (SNV)
- Initiates and completes professional nursing tasks based on a person’s assessed need for services to maintain or restore optimal health
- Happens at a person’s place of residence or in the community where normal life activities occur
- Happens on an intermittent basis by an RN or LPN under the supervision of a RN
- May be conducted in person or via telehomecare technology.
. Persona Care Assistance (PCA)
Services to help people with day-to-day activities in their home and community to help a person maximize his/her independence.
. Medication administration and counselling
Well trained staff of Presence Homecare provide medication administration and counseling by pharmacists to all the medications residents are taking. We also coordinate medications across all physicians to ensure compatibility and efficacy.
